What is modifier downcoding?
Modifier downcoding is when a payer ignores or removes a modifier the provider billed, which lowers or denies the payment for the service.
Modifiers such as 25 (a significant, separately identifiable evaluation and management service) or 59 (a distinct procedural service) tell the payer a service should be paid separately or differently. If the payer processes the claim as if the modifier were not there, the service may be bundled, reduced or denied.
Modifier-related underpayments are easy to miss because the claim still shows as processed.
How to spot it
- Check whether services billed with modifiers 25, 59 or the X-modifiers were paid separately, or were adjusted as included in another service.
