What is downcoding?

Downcoding is when a payer pays a claim as if a lower-level or less complex service code had been billed than the one the provider submitted.

A common example is an evaluation and management visit billed at one level and paid at a lower level. The payer may do this through automated claim edits rather than a review of the medical record.

When documentation supports the billed code, the difference can be appealed.
