What is coordination of benefits (COB)?

Coordination of benefits is the process payers use to decide which insurer pays first, and how much the second insurer owes, when a patient has more than one plan.

The primary payer pays first; the secondary payer then pays according to its own rules and the primary payment. Errors in either step, such as the wrong primary payer or a secondary payment calculated from the wrong amount, can leave a claim underpaid.
